Media Release Form
For over 26 years we have facilitated local community events which may be visually chronicled for future articles, marketing and promotional use. The form below registers the approval of an individual’s permission to be included in photography or video footage. Children under the age of 18 require permission from a parent or guardian for their faces, names or other identifiable features to be captured in photography or video footage at community events.
By completing this form, I grant permission to The Top Ender Tri-Services Magazine Inc. (The Top Ender) to use photographs, video images and comments of the individuals listed below in print and broadcast media. I acknowledge The Top Ender Tri-Services Magazine Inc. may alter, crop or treat the photograph/video at its discretion.
In addition, I waive any right to inspect or approve the finished product, including written copy, wherein these individual’s likeness or comments appear. I also acknowledge that The Top Ender Tri-Services Magazine Inc (The Top Ender) may choose to use photo/video/comments at this time but may do so at its discretion at a later date.
The Top Ender Tri-Services Magazine Inc (The Top Ender) reserves the right to discontinue the use of photos/ videos/comments without notice.
General Guidelines: It is recommended that approval be obtained when photographing or videotaping a minor (under 18). A parent or guardian signature is required; signatures of minors are not sufficient. When images are published, The Top Ender Tri-Services Magazine Inc (The Top Ender) will take cautionary steps to provide minimum identifying information and will not use specific street or mailing addresses, e-mail addresses, or phone numbers. Photographs or video images during a photo or video shoot organised by The Top Ender Tri-Services Magazine Inc (The Top Ender) must be done only with The Top Ender Tri-Services Magazine Inc (The Top Ender) permission and with signed release forms from a parent or guardian of each child. It is the responsibility of The Top Ender Tri-Services Magazine Inc (The Top Ender) to obtain signed release forms and maintain records.
